1. The application of the established form (you also have to have the passport with you);

2. Color photos 3х4 - 3 pieces;

3. An envelope;

Payment function: for the state certificate for the right of hunting and an account card of hunting rules violation - 100 000 rubles.

GLHU "Polotsk forestry enterprise"
"BelAPB" Polotsk, code 442, s/a 3605900130014;
Payment function: for the hunting examination - 0,2 base amount (20 000 rbl.).

ATTENTION!!! It is paid at a branch of the savings bank reception while getting the state certificate:
RAIFO

Payment function 03002: the state tax for the hunting right - 1 base amount (100 000 rbl.).
